I had the same thought as Linda. You can always call Google Support and ask them whats going on. There's a 50/50 chance that you will get someone that can help shed some insight and they may even fix it for you.

A little late to the party but I will put my 2 cents in anyway. I have noticed that, not only what Linda said earlier about the keyword density, but sites that tend to rank in the maps pack usually have the Google+ page linked in some way. Either through the publisher tag hidden on their site...

I'm located in Salt Lake City, UT. I've tested with Chrome, Firefox and, as painful as it was, IE. All show the same results depending on whether you are logged into Google or not.
