I have seen a few cases where professionals (e.g. Doctor, Physical Therapist) have a "personal"/professional GBP with the primary category being "Physical Therapist" or "Men's health physician" in addition to a GBP for where they practice with a category like Physical therapy clinic or Medical Clinic.

My question:
What is the most successful way to create and maintain the GBP for the individual professional *and* the offices where they practice?
 
I wouldn't do it. In almost every case I've worked on, the practitioner listings add more complexity and very little benefit.
