Strange errors on Google+ pages verified to merge with Google+ Local...

I have noticed a few times recently that a company will show up on a local search with 2 listings, 1 from the Google Places side and 1 from the Google+ page. The Google+ page is verified but did not merge to the Google Places listing.

Has anyone else seen this?

Clearly when a PIN has been entered on the Google+ page it should complete the merge and only 1 listing should appear on the local search.

It makes me wonder if it is an error by the people optimizing the listing or just an error by Google.

Thoughts?

That problem has come up on the Google forum A LOT and is one of many reasons I don't recommend merging yet.
