Ads in top 4 plus bottom of page?

Hi there

Two of my client's competitors have ads that appear twice on the first page of the SERPs: once in the first four ads at the top of the page and again at the bottom of the page.

they are not in a competitive industry - there are about 4-5 companies that offer this service in total in our city

Ideas as to how to achieve that? BTW notice I stress top 4 and not the ads in the local pack as we show up there on occasion.
