How viable is it to repurposed an existing well reviewed and aged GBP for a new address, but as a practitioner listing? The clinic name is the doctor's, so it would remain the same at the new address.

The goal here is to leverage the existing listing reviews to get a head start in the new location.

Since they’re now serving both states, can we set up a brand-new GBP for the original location?
 
why don't you create a new one for the new address? why go in a hard way? you can create a fresh legal GMB on your new place and keep the old one as it is. as you are serving both states you can keep the name same as long as it is legal
 
I think it could work, but I wouldn't risk it. Especially considering you plan to keep the existing listing.

Changing important details like address and primary category can put a target on your GBP. It could get temporarily suspended or worse.

I think most of us who have managed hundreds of GBPs are cautious with big changes. Most of the time it goes smoothly, but when it goes bad it can be a nightmare.
