Review Pages

I am in the process of setting up a page for customers of a client to use to click through to leave a review at one of the major review sites, but am not super fond of the layout yet and wanted to see if anyone out there has any great examples of pages that drive customers to leave reviews that has been effective.

Also, have a look at BrightLocal - If you look on their services page, they have "ReviewBiz" - a service that gives you review badges to put on your site to drive up reviews.

Thanks Linda and Nick both great resources. I had seen that Brightlocal review thing a while back but had completely forgotten about it.
