Business name question / Name Spam

I'm doing local SEO for a dentist and her business name is her own name (First name + Last name).

Do solid citation building + GMB optimization + Couple reviews can beat Name spam on Google map today ?

If I ask , it's because on the local map pack, each top competitor included keywords on their name. It's like "Dentist - Doctor X - City" or only "Dentist - Doctor X".
Is it wrong to do the same for my client ?

According to the guidelines, you aren't supposed to add the city to the title unless it's a part of the actual business name.
