I don't use twitter for my clients but am curious is setting up a Twitter Card is advisable for those that do use Twitter and might possible share my clients website with others. Also, I assume I would need to set up a Twitter account for my clients in order to set up a Twitter Card?

Is this advisable or a waste of time?

If you're talking about the OpenGraph, yes, set them up on your site. Set it up for both Twitter and Facebook.

As far as your client not using Twitter, I would encourage them to snag their name on there nonetheless - do it with the major platforms; Twitter, Facebook, Pinterest, etc. While they may not be used, you're at least stopping someone else from snagging your client's business name.
