1- it was independent of whether you accepted them or not. The feature on suggested edits is opt out not opt in and it was the suggestions that caused the issue.

2- it appears to have been fixed per the Google Search Liaison and the fact that no one seems to have reported in the forum of late.

It appears they went back and fixed some of the ones that they buggered up. But if it occurred in August, which some did, those seem to have been lost forever.
