Least Expensive Service for Issuing/Tracking Feedback Requests

A lot of these sites like Grade.us are overkill for what we need. We like to encourage our clients to tell us how we did. Obviously we could just maintain a list of email addresses in a spreadsheet or word doc but that's pretty clunky and won't be efficient as our client base continues to grow. We don't necessarily want to directly solicit 5-star reviews from people as that's against policy but we'd like to make it as easy as possible for people to learn how to do it should they choose.

Phil Rozek's free handout generator comes somewhat close to what we're looking for but we wouldn't mind paying a small fee to get some rudimentary management features. Look's like reviewpush.com is currently the cheapest amongst these types of services at $25/month per location. Any other alternatives to consider that aren't more expensive?
